<p class="abstract">Angiolymphoid hyperplasia with eosinophilia (ALHE) and Kimura’s disease are uncommon chronic benign lesions of the skin mainly involving head neck regions where ALHE involves vascular lymphocytic proliferation is inflammatory type. Dermal involvement more in but In lymphnodes subcutaneous tissues involved more. a rare tumor disease. Both not only have uncertainty etiology pathogenesis, also there therapeutic dilemma management. this article, we describe case which presented to our ENT outpatient department swelling left postauricular region appeared 1.5 years back posed riddle coming correct diagnosis. On examination, single smooth spherical upper part post-auricular size 3×3 cm well-defined edges normal skin. Blood investigations revealed elevated absolute eosinophil count. HRCT temporal bone showed rounded hypo-dense lesion measuring 28×23 mm no effect adjacent bony erosion / calcifications/ cystic changes suggestive etiology. FNAC scantly cellular occasionally scattered fibroblasts, pinkish stromal collagen fragments cells background haemorrhage soft tissue neoplasm.</p>
